Small Fiber Neuropathy

Abstract
Small fiber neuropathy is a clinical condition which affects small caliber peripheral nerves i.e. A¥ä and C fibers. Main clinical characteristics of small fiber neuropathy are neuropathic pain and autonomic symptoms. Diagnosis of small fiber neuropathy is challenging due to absence of a gold standard diagnostic test, so the diagnosis is based on typical small fiber neuropathy related symptoms, normal nerve conduction studies, reduced intraepidermal nerve fiber density at the ankle, and/or abnormal quantitative sensory testing. Although small fiber neuropathy may be assocatiated with various systemic diseases, this neuropathy remains idiopathic in a large portion of patients. Recently, gain of function Nav1.7 mutation has been found in about 30% of patients with idiopathic small fiber neuropathy. Because there are several treatable disorders associated with small fiber neuropathy, the correct and early diagnosis of small fiber neuropathy and the effort to find the underlying disorders are important.
